FinalForms is now open for the 24-25 school year for you to complete your PK-12 school forms and athletic forms online at your convenience.

If you are new to Northmor Schools, you will need to call the school office to register. After registration with the school secretary, you will receive an email with a link to FinalForms. Please note that school offices do not open until the middle of August. (see office hours at the end of this article) If you are new and planning to play fall sports in grades 7-12, please email Mr. Tackett, Athletic Director, at btackett@knightpride.org.
Fall Athlete Instructions:
Any athlete that will be participating in a fall sport needs to complete all their paperwork within FinalForms, AND also select the fall sport they will be playing. If you plan to play a winter or spring sport, you can select those sports as well.
Please remember this is a two-step process where the parent/guardian signs into FinalForms and selects the various sports their child will participate in, and then completes all the required paperwork on the parent/guardian end. After the parent/guardian completes their portion, an email will be sent to the student letting them know it is time for them to complete their forms. At that time, the athlete will log in, and complete their required paperwork using their own email and password. *Student’s knightpride email address can be used with FinalForms
All athletes must have a current physical form on file in the athletic office. This form needs to be printed directly from FinalForms and taken to your doctor to be completed. This form is the last one on the left-hand side of the page that you will sign off on. Please remember that you can also check the date as to when your most recent physical will expire, for those that have played a sport in the past. After getting your updated physical, upload the doctor-signed forms back into FinalForms, turn in the form to your fall coach or turn in the form to the Board Office during summer hours.
